Chris Small, M.D

Addiction Psychiatrist, President Headlands ATS
Dr. Small received his medical degree at the University of Hawaii. He completed his medical residency in Psychiatry and Family Medicine at UCSD. He is board certified in Psychiatry, Addiction Medicine, and Family Medicine. Dr. Small is passionate about bringing quality care to patients suffering with addiction.

Jena Horn, BBA

Operations Manager
Jena has several years of experience in operations, human resources, business administration and management. She is a graduate of the Univerity of New Mexico. She brings excellent customer service, organization, and liaison/support to our program partners every day.

Weekend Triage Nurse

Amanda completed her nursing degree at the University of Louisiana at Lafayette. She has a decade of experience in the field of nursing ranging from chemical dependency, mental health, neurology, and occupational medicine

Mari Hanson, PA-C

Physician Assistant CAQ Psychiatry
Mari has 19 years of experience as a Physician Assistant. She has a diverse background in medicine which includes Primary Care, Integrative Medicine, Psychiatry and Addiction medicine and Hepatitis C treatment. She graduated from Red Rocks Community College Physician Assistant program in Lakewood, Colorado in 2001. She obtained her Certificate of Added Qualification in Psychiatry in 2020.

Kelly Mullins, PMHNP

Advanced Practice Supervisor
Kelly has a long history of service. She served as an Army Healthcare Specialist and medic in Iraq and Honduras and as an RN at UCSD prior to achieving her designation as a Family Nurse Practitioner from Azusa Pacific University and Psychiatric Family Nurse Practitioner at University of Cincinnati. She has extensive experience in primary care, psychiatry and addiction medicine.

Diana Pasquarello, PA

Physician Assistant
Diana has 17 years of experience as a Physician Assistant in primary care and addiction medicine. Diana completed her undergraduate degree at UCSD and received her Master’s in Physician Assistant in 2003 from USC. She is passionate about improving access to Medication Assistant Treatment (MAT) in patients suffering from opiate dependence.

Olga Jackson, FNP-BC

Family Health Nurse Practitioner
Olga has 10 years of nursing experience, receiving her RN in 2011 and Family Nurse Practitioner designation in 2018 from South University. She has extensive experience in behavioral health and addiction medicine. She is currently participating in her clinical requirements for PMHNP designation.

Megan Duggins, SHRM-CP

Human Resources Manager
Megan brings to Headlands extensive experience as a human resources representative at the University of Chicago and as an advocate/outreach director for disadvantaged children. She holds a masters degree in Industrial Organizational Psychology from Emporia State University.

Halee Fullerton, PMHNP

Psychiatric Nurse Practitioner
Halee obtained her nursing at the University of Texas at Arlington, and her masters in psychiatric nursing in 2018 at the University of Missouri- Kansas City. Her nursing experience includes 8 years of ER, ICU, med-surg, and dementia unit service. Prior to joining Headlands, Halee provided psychiatric services in the community health setting for children, adolescents and adults.

Debra Lloyd, PMHNP

Psychiatric Nurse Practitioner
Debra has devoted her entire nursing career to working with psychiatric and chemically dependent patients. She is a graduate of Mount St. Mary’s college and received her Masters degree at Maryville University. In addition to her certification as a psychiatric nurse practitioner, Debra carries certifications in addiction nursing (CARN) and public health nursing (PHN).

John Hosley, PA

Physician Assistant
John completed his undergraduate work at UC Davis and Master Degree of Physician Assistant Studies at University of the Pacific. A long time resident of the Sacramento area, John is committed to providing excellent, compassionate patient care in his community.

Carrie Starkey

Office Associate
Carrie brings several years of experience in human resources with a focus in staff development and learning management. She holds a Masters degree in Industrial and Organizational Psychology from Emporia State University.

Dr. LaFrenierre

Psychiatry Director

Dr. LaFrenierre received her medical degree at University of California, Los Angeles and completed her psychiatry residency at San Mateo Behavioral Health and Recovery Services. She also completed a fellowship through the Harvard Program on Refugee Trauma, and has expertise in the treatment of PTSD, veteran and refugee populations. She is board-certified in Adult psychiatry, and board-eligible in the specialty of Addiction Medicine. She has had a variety of specialty training experiences in addiction, geriatric and forensic psychiatry, and is committed to providing empathic and comprehensive care to her patients.

Jamie Guthrie RN, MSN, APRN-CNP

Jamie became a Registered Nurse in 2009, and achieved her Masters of Science as a Family Nurse Practitioner in 2017 at Graceland University. Her nursing experience includes 12 years of emergency medicine, and psychiatric care. Jamie is very passionate about empowering her patients to take an active, and well-educated role in their healthcare.

Paul Edwards,
MSN, APRN, FNP-C, EMT-LP

Family Nurse Practitioner

Paul Edwards is a Certified Family Nurse Practitioner, member of The American Association of Nurse Practitioners, training center faculty for the American Heart Association, instructor for the American Heart Association, and a Licensed Paramedic in the state of Texas. He specializes in the pharmacological management of psychiatric disorders and addiction medicine. Paul began his medical career as an EMT-basic and quickly became a Licensed Paramedic in Texas with nearly 10 years of emergency medicine experience. He attended the University of Texas at San Antonio (UTSA) where he obtained a Bachelor of Science in Biology. While at UTSA, Paul was involved in multiple honor societies such as Alpha Chi National Honor Society in which he served as president, Student Representative for Region 1, and Board Member. He then attended the University of Texas at Arlington, where he obtained his Bachelor of Science in Nursing and became a registered nurse. He then went on to attended Herzing University, where he obtained his Master of Science in Nursing with a Family Nurse Practitioner specialization. Throughout his many years of medical expertise, Paul has been able to take his hands on experience, knowledge and dedication to provide his patients with the highest standard of care. He is a firm believer in treating the patient, not the diagnosis alone.

Danielle Lacy

Physician Assistant

Danielle has over 10 years of experience providing medical care and patient advocacy. Danielle completed her undergraduate degree in Psychology and Public Health at Spelman College in 2012 and received her Master’s in Physician Assistant Science from Samuel Merritt University in 2019. Her medical background includes working in Women’s Health, Hospital Medicine, Addiction Medicine, and Public Health. 

Zhanette Coffee, FNP-C, MSN, APRN-CNP

Zhanette has a broad background in chronic pain, substance use disorders, and mental health. She also has experience working as a clinical researcher, clinical nursing faculty, First Responder Police Officer, Lead Intensive Care Registered Nurse, Clinical Nursing Faculty, and Lead Public Health Nurse. Zhanette carries a Master of Science in Nursing – Family Nurse Practitioner – from Azusa Pacific University. She is currently pursuing a Doctor of Philosophy (Ph.D.) in Nursing student at the University of Arizona with a scholarly interest in investigating the best-practice treatment approach to the phenomenon of addiction and chronic opioid analgesic therapy reliance in chronic non-cancer pain patients. She is passionate about education, healthcare policy, holistic nursing, and motivated by core values that exemplify not only hard work, but honestly, humility, and gratitude.
